Indonesian immigration offices photograph applicants during the interview, so the submitted print is used for the application file rather than for the passport page itself. Bring it anyway — applications are turned away without it.
The 4 x 6 cm format is the same size used across many Indonesian administrative forms, so the sheet is reusable. Note that some forms ask for a red or blue background instead — check the specific form, as the passport application itself wants white.
